New ICD-10-CM Codes for FY 2023 Show Sizable Increase

Change is an evitable part of life and a constant in healthcare. One thing medical coders can count on is change.

CMS has released significant ICD-10-CM changes for the fiscal year 2023 (FY 2023), with 1,176 new codes being implemented. That’s a whopping increase over the 159 new codes added for FY 2022. Furthermore, for FY 2023, 287 codes will be deleted, and 28 code descriptors will be revised.

These code changes take effect on October 1, 2022, and will remain in effect through September 30, 2023.

ICD-10-CM changes for FY 2024 are just around the corner and will take effect on October 1, 2023, which I have discussed in a recently published article.

Highlights of ICD-10-CM Code Additions By Chapter

Most of the chapters in ICD-10-CM had new codes added for FY 2023, with the majority occurring in Chapter 20: External causes of morbidity. Many of the additions in this chapter are associated with motorcycle and electric bike accidents.

Other significant code additions pertain to maternal care for fetal disorders (Chapter 15), endometriosis (Chapter 14), head injuries (Chapter 19), and dementia (Chapter 5).

Highlights of the code additions for each chapter are explained in this article. Samples of new codes are also provided. However, they are only a portion of the codes released.

The FY 2023 coding guidelines will also take effect on October 1. The coding guideline changes are in bold to help the coder identify the new and edited information.

Chapter 1: Certain Infectious and Parasitic Diseases (A00-B99) 

  • Candidiasis of vulva and vagina has been expanded as to type

Code additions:

  • B37.31, Acute candidiasis of vulva and vagina
  • B37.32, Chronic candidiasis of vulva and vagina

Chapter 3: Diseases of the Blood and Blood-Forming Organs and Certain Disorders Involving the Immune Mechanism (D50-D89)

  • Hemolytic-uremic syndrome
  • Von Willebrand disease codes have been expanded based on type 
  • Thrombocytopenia

There is also a new code for Activated Phosphoinositide 3-kinase Delta syndrome [APDS] (D81.82).

Chapter 4: Endocrine, nutritional and metabolic diseases (E00-E89) 

  • Short stature due to endocrine disorder
  • Acidosis based on the type

  • Dementia has been expanded to indicate types by severity. These new codes also provide additional detail on other key associated disorders.

Chapter 13: Diseases of the Musculoskeletal System and Connective Tissue (M00-M99)

  • Intervertebral annulus fibrosus defect (lumbar and lumbosacral region based on size)
  • Muscle wasting and atrophy (back based on region)
  • Slipped upper femoral epiphysis (nontraumatic) for stable and unstable conditions based on severity and laterality. The word “stable” has been added to existing codes for upper femoral epiphysis.
  • Fracture of sternum, one rib, or multiple ribs associated with chest compression and cardiopulmonary resuscitation (CPR)
  • Flail chest associated with CPR

Codes from category O35 require a 7th character to identify the number of fetus affected. A 7th charter of “0” means a single gestation; “1-5” means multiple gestations; “9” means multiple gestations greater than five. For example, O35.05X2 would be reported for maternal care for (suspected) central nervous system malformation or damage in fetus, holoprosencephaly, fetus 2.

Chapter 16: Certain Conditions Originating in the Perinatal Period (P00-P96)

  • Sleep apnea in newborn children based on the specific type

Chapter 20: External Causes of Morbidity (V00-Y99)

Chapter 21: Factors Influencing Health Status and Contact with Health Services (Z00-Z99)

Six Chapters With No Code Additions

There were no code additions made to the following chapters for FY 2023:

  • Chapter 2: Neoplasms (C00-D49)
  • Chapter 7: Diseases of the eye and adnexa (H00-H59)
  • Chapter 8: Diseases of the ear and mastoid process (H60-H95)
  • Chapter 12: Diseases of the skin and subcutaneous tissue (L00-L99)
  • Chapter 18: Symptoms, signs, and abnormal clinical and laboratory findings, not elsewhere classified (R00-R99).
  • Chapter 22: Codes for special purposes (U00-U85)

Although Chapter 2 did not include any new codes, there have been 11 changes made to codes for peripheral T-cell lymphoma. Instead of saying “not classified,” the descriptors now say “not elsewhere classified.”

Chapters 7, 8, 12, 18, and 22 had no code additions, changes, or deletions.
